The patient was operated 20 years ago for PLD L4-5. He started to complain of left sciatica for 5 months with drop foot . MRI showed moderate extrusion L3-4  left side with segmental stenosis, for what laminectomy L4 and Partially of L3 was done with foraminotomy left L4 root was done and the extrusion was removed and moderate cleaning of the disc space was performed.
