EXTRUDED DISK L1-2 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
24-May-2016 complaining of LBP with left sciatica for
1 month down to the gastrocnemius muscle. MRI lumbar spine performed
10-May-2016
showing huge disk L1-2 with left downward
migration.
On examination: the patient is limping with
mild scoliotic
stance. SLRS was 30 degrees in the right with
pain shooting to the left leg and 60 degrees left side with
less pain. There is weak
dorsiflexion left foot -4/5 and dyseasthesia of
the left L1 territory.
Using the C-arm, the
L1-2 level was identified and left L1
foraminotomy was achieved. The extruded disk was removed from
left side and left sided cleaning of L1-2 was
performed. Using MultiGen, stimulation of left nerve was achieved with
0.8 V. A
bipolar pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration to left L1 root was achieved using 2 bended catheters 10 mm exposed length. Further
motor stimulation done to the same root and the
same response was noted. Routine closure of the
wound.
Smooth postoperative recovery. The patient
is sciatica free. The power of left foot
normalized and he was sent to the ward.

Comments
The patient still have an estimated
postoperative recurrence around 7%, because the disc space
is still not shallow.
This is the 59th case using the BPRF mode
with MultiGen. This procedure regained routine acceptance it
became
a usual part of the spine surgery.
