The patient started to complain of left sciatica for 8 months and MRI performed 20-March-2005 showed huge disc L4-5 with left downward migration. The patient came to the clinic with agonizing pain and right sciatica for 4 days. On examination, he had weak dorsiflexion both feet and planterflexion left foot. New MRI performed, which showed that a fragment in the lower part shifted to the right. The patient underwent bilateral flavotomy L4-5 with bilateral foraminotomy L5 and the extrusion was removed from the left side and bilateral cleaning of L4-5 disc space was performed. Inspection of the right L5 root confirmed that the lately dislodged part was removed with the major big extrusion in one piece.

Prompt postoperative recovery and the patient discharged the next postoperative day without sciatica and full power of his feet.
