DISC C5-6 WITH RIGHT FORAMINAL OCCLUSION AND BULGE DISC C6-7.
Anamnesis
The patient came to the clinic 24-March-2018
complaining of neck pain for 1 week with right
upper limb pain with numbness of
the right hand the last day. The patient performed MRI
cervical spine 24-March-2018 showing extruded
disc C5-6 with right foraminal occlusion and
bulge C6-7. The patient underwent transpedicular
fixation L3,4,5 2013 elsewhere with broken lower
screws.
On examination; the patient has severe pain when
turning the head to both sides more to the
right, and up. Turning the head to the chest
relief the pain. There is weak grip and extension of the right hand 4/5
and right triceps 4/5. There is hypalgesia
little finger right hand. There is weak
dorsiflexion both feet 4/5.
The patient was sent for investigation and lab
investigations were normal.
Discectomy C5-6 until the dura was seen at both
sides. ON PLUS (EOS) Cervical
bladed cage 14x17 H7 with IN'OSS 2x0.5 ml inserted to C4-5 level. Routine closure of the
wound.
Smooth postoperative
recovery. The power of the right upper limb
normalized and the radicular pain disappeared. He was sent to the ward.
Comments
The patient has an extruded disc
which is not subsiding. Surgical decompression is the only
solution.
