NAHI ABDALLA AL-SHEIKH 61 YEARS SPONDYLOLISTHESIS L1-2, L3-4 WITH SEVERE
STENOSIS L1-2 DOWN TO L4-5.
Anamnesis
The patient came to the clinic
22-December-2015 complaining of LBP for 15 years
with right sciatica. Exacerbation of right
sciatica the last 2 months down to the right
gastrocnemius muscle with numbness of this area.
She is a known hypertensive for 8 years in
hypoten 100 mg once daily.
On examination: the patient is limping for 10
years with exaggerated scoliotic
stance. SLRS was
90 degrees both sides without pain. There is weak
dorsiflexion right foot -4/5. There is severe OA
both knees with pain and limitation of flexion
right knee.
The patient was sent for investigations and MRI
lumbar spine showed severe stenosis L1-2 down to
L4-5 with scoliosis and spondylolisthesis L1-2
and L3-4. Right knee synovitis with torn medial
meniscus. Dynamic X-rays confirmed the data
acquired with MRI.
The patient then came 30-April-2016 claiming
that the right knee improved. and sent for
cardio evaluation. MRI repeated 04-May-2016
which showed the same data as before in the
lumbar spine.
Decompressive laminectomy of L1
down to the upper
half of L5 was
achieved. Foraminotomy both L1,2,3,4 and L5 roots was done.
Using Expedium spine system 10 monoaxial screws
6x45 and bended 2 rods, 15 cm length and cross
connector medium, were used to have
transpedicular correction and fixation of
L1,2,3,4 and 5. Stimulation of screws before
connecting them with 8V stimulation did not show
any radicular responses. All stages of
surgery were guided with C-arm. Using MultiGen, stimulation of the
right L3,4,5 roots was achieved with 2 V. A
bipolar pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration to right L3,4,5 roots was achieved using 4 bended catheters 10 mm exposed length. Routine closure of the
wound.
Smooth postoperative recovery. The patient
is sciatica free. The power of right foot
normalized and she was sent to the ward.

Comments
This is the 55th case using the BPRF mode
with MultiGen. This procedure regained routine acceptance it
became
a usual part of the spine surgery.
