The patient OF normal delivery , premature , born and was in incubator for one month. CT-scan done at that time showed diffuse brain atrophy over the frontal lobes without evidence of hydrocephalus. The patient over the last 4 months started to show growing size of the head and bulging fontanel. MRI performed the last two days showed progression of the hydrocephalus with still widened subarachnoid spaces over the frontal lobes. So as to resolve the problem with the ventricular and subarachnoid spaces, the ventricular end was performed with 70 mm depth and three miniholes were created directly under the level of the dural cleavage, so as to have communication between the ventricular and subarachnoid spaces and to flow through the shunt. You can notice that first degree of Arnold-Chiari malformation having place, which could be the precipitating factor.
