Anamnesis
The patient came to the clinic 17-January-2005
complaining of right
sciatica for 4 months with positive cough sign.
The condition is deteriorating. MRI done
17-January-2005 showing huge extruded disc L4-5
more to the right.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion both feet 4/5. SLRS was
20 degrees with pain right side.
Extended right L5 foraminotomy
with removal of the extrusion and right sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 30-January-2005
with SLRS 90 degrees in the right with
disappearance of hypalgesia and no motor
deficit.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.

Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
