01-JULY-2003 ABDALLAH ALI SAYYAN 57 YEARS EXTRUDED DISC L4-5 LEFT SIDE.
Anamnesis
The patient an Iraqi citizen came to the clinic
29-June-2003 complaining of LBP for 3 years with
exacerbation of LBP with left sciatica the last
month. MRI done recently showing extruded disc
L4-5 with left foraminal occlusion.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 80
degrees both sides with pain in the left. There
is weak dorsiflexion left foot 4/5.
Left L4-5 hemiflavotomy with
foraminotomy left L5 root. The extruded disc was
removed lateral to the left axilla and left
sided intradiscal cleaning L4-5 was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed improvement of the power of
his left foot.
He was sent to the ward.
FOLLOW UP
The patient
did not came up for follow up.
Comments
There is still an estimated postoperative
recurrence less than 7%, because the disc space
is shallow, but not completely collapsed.
