26-APRIL-2002 ABDALLAH ALI MUSA AL-ZAWAYDEH
27 YEARS SEVERE HEAD INJURY.
Anamnesis
The patient was brought to the ICU of Shmaisani
hospital 26-April-2002 with severe head injury
with right subdural hematoma , contusion with
edema right cerebral hemisphere and fracture
right fronto-temporal bones.
On examination: The patient is responding with
difficulty to verbal command with gross paresis
left side of the face. Serial CT-scans were the
same. The morning of 27-April-2002 showed
deterioration of his condition with gross left
sided plegia more the left upper limb with
deterioration of the level of consciousness.
The patient at the evening of 27-April-2002
progressed convulsion after what plegia of the
left lower limb took place.
The patient started to show
improvement of his condition with regression of his
neurologic deficit and the left sided plegia
considerably regressed without any surgical
intervention.
In 01-May-2002, the patient
continued to improve to show elements of frontal
lobe syndrome of minor severity. The patient was
discharged 05-May-2002 and disappeared.
Comments
This case s a demonstration, that not
every severe head injury needs surgery. Control CT-scan and
preferable MRI with certain manifestations, such as pending
conning must change policy and hurry with surgery.
