43 YEARS GIANT EXTRUDED DISC L5-S1 WITH RIGHT UPWARD MIGRATION.
Comments
The estimated postoperative recurrence is
around 7%, because the disc space is still not completely
shallow.
Anamnesis
The patient was hospitalized to the Emergency of
Shmaisani hospital with agonizing LBP with drop
right foot. SLRS was 10 degrees right side with
pain. MRI lumbar spine performed 28-May-2003
showing huge extruded disc L5-S1 more to the
right.
Bilateral L5-S1 flavotomy with
foraminotomy both S1 roots. The extrusion was
attacked first from the right side lateral to the
right S1 root and a very huge piece of extrusion was
removed. Right sided intradiscal cleaning done.
Inspection of the left side was uneventful. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery. He was sent to the ward.
Follow Up
The patient came to the clinic 10-June-2003 with
clean wound with weak dorsiflexion right foot 4/5. SLRS
was 50 degrees with pain in the right.
