YEARS RECURRENT CHORDOMA AND FRACTURE OF THE PELVIC RIM.
Anamnesis
The patient a Syrian citizen was operated by me
twice and the last one in
22-January-2003 for recurrent chordoma of
the sacrum and iliac bones. The patient walk
with help and felt that the right hip and right
sciatica increased the last month.
On examination, the patient in pain,
limping with exaggerated scoliotic stance with
weak dorsi and planterflexion both feet, more the
right and hypalgesia
both L5 and S1
territories. SLRS was 30 degrees right side with
pain.
The patient was sent for
investigations and MRI lumbar spine performed
10-April-2004 showing recurrence recurrence
chordoma and broken construct and the right rim
of the right iliac bone.
Exposure of the right rim of
the iliac bone and identification of the
fracture site and removal of the accessible
parts of the tumor. Using Hartshill rectangle
bended and modified to accept the area, fixation
of the fractures area and the previously
implanted device with sublaminar wires was
achieved. Routine closure
of the wound.
Smooth postoperative recovery. The power of both
feet the same.
FOLLOW UP
The patient
came to the clinic 23-April-2004 with clean wound
and the power of the feet the same
and SLRS was 80 degrees both sides without pain.
The hypalgesia is the same, but the right sciatica
and right hip pain disappeared.
Comments
This is clinically not
chordoma, but mostly chondroblastoma in
behavior.
