21-JUNE-2015 ABDELHAMEED BASHEER ALDUWAYK 81 YEARS
CERVICAL CANAL STENOSIS C3-4, C4-5 AND C5-6 WITH MYELOPATHIC SYNDROME.
Anamnesis
The patient came to the clinic 30-September-2014
complaining of left shoulder pain for 2 months,
more during night. MRI investigations performed
29-September-2014 showing stenosis at C3-4, 4-5
and C5-6 with left shoulder supraspinatus
tendinitis. On examination at that time there
was weak left deltoid and triceps muscle 4/5
with frozen left shoulder. He was treated
conservatively. The patient then came
22-April-2015, limping as dragging the left leg
for 3 months. MRI lumbar spine performed
04-April-2015 showing spondylolisthesis L4-5.
On examination; the patient is limping with
ataxic gait. There is pain when turning the head
to the left and flexion to the chest, with weak
extension of the left hand 3/5 with hypalgesia
of the left median distribution. SLRS was 60
degrees with pain both sides. There is weak
dorsiflexion both feet -3/5 and planterflexion
left foot 3/5 with hypalgesia below left L4
root.
The patient was sent for new investigations and
MRI of the brain showed a small convexital
meningioma 11x11 mm diameter of no clinical
importance. MRI of the cervical spine showing
the severe cervical stenosis C3-4, 4-5 and C5-6
with malacia of the spinal cord.
Decompressive laminectomy of C3-4-5 and C6 in
one block after drilling the sides abutting the
lateral masses. The block was removed in one
piece. Routine
closure of the wound.
Smooth postoperative recovery. The power of
both feet became better.
Comments
The patient has cervical canal stenosis
which was clinically silent. After progression surgery was
advised.
