EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 01-June-2015
complaining of LBP for15 days with
exacerbation of right sciatica for 4 days down
to all toes right foot. MRI lumbar spine done
28-May-2015 showing extruded disc L4-5 with
right downward migration.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 20
degrees with pain right and 90 degrees in the
left. There is weak dorsiflexion
right foot 3/5.
Using the C-arm, the level of L4-5 was
identified and foraminotomy of right L5 root
performed. The extruded disc was removed lateral
to the axilla. Inspection of the subaxillary
area for remnants. Right sided intradiscal
cleaning L4-5 disc space. Routine
closure of the wound.
Smooth postoperative recovery. The power of
right foot became normal.
Comments
The patient still having an estimated
postoperative recurrence around 7%.
