YEARS HUGE EXTRUDED DISC L4-5 WITH RIGHT POSTERIOR MIGRATION.
Anamnesis
The patient came to the clinic 19-October-2013
complaining of LBP without sciatica took place 2
years ago, then resolved. The LBP then took
place 17-September-2013 with right sciatica down
to the little toe right foot with numbness and
positive cough sign.
MRI lumbar spine done 09-October-2013 showing
huge extruded disc L4-5 with right posterior
migration.
On examination: the patient is using crutch for
10 days, limping with exaggerated scoliotic
stance. SLRS was 10 degrees with shooting pain
in the right and 30 degrees with pain radiating
to the right. Weak dorsiflexion right foot 3/5
and planterflexion 4/5.
Right L4-5 hemiflavotomy with foraminotomy right
L5 root. The extruded disc was removed lateral
to the axilla and right sided cleaning L4-5 disc
space was achieved.
Smooth postoperative
recovery. The power of the right foot became
normal.
Comments
The patient still have an estimated
postoperative recurrence rate around 7%.
