YEARS HUGE EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 30-October-2014
complaining of LBP for 2 weeks with agonizing
right sciatica for the last 6 days. MRI lumbar
spine done 28-October-2014 showing huge extruded
disc L4-5 with right downward migration. The L5
is sacralized. The patient receiving Augmentin
625 mg three times daily for 2 weeks for
tonsillitis.
On examination; there is limping with
exaggerated scoliotic stance. SLRS was 15
degrees with pain in the right side with absent
AJ in the right. Weak dorsiflexion right foot
-3/5 and planterflexion same foot 4/5.
Using C-arm and MICRODISCXS, the L4-5 level was
identified and foraminotomy of the right L5 root
was performed. The huge extruded disc was
removed from under the axilla and subaxillary
cleaning of L4-5 disc space was achieved. It was
impossible to capture events without VITOM, for
what during video recording the tubular system
was removed.
Routine closure of the
wound.
Smooth postoperative recovery.
The power of the right foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not completely shallow.
