YEARS PARAPLEGIA BELOW C8 LEVEL.
Anamnesis
The patient came 24-November-2003 telling that he suffered RTA 2
years ago with fracture C6,7 with paraplegia at these levels. MRI
done 29-November-2001 showing bad alignment of C6 and C7 with cut of
the spinal cord. He had fracture of the right parietal bone with
bone defect at that area with convulsions. He was not covered with
anti-epileptic drugs and in Foley's catheter. He was operated
02-December-2001 and posterior fixation was done for him using
Hartshill rectangle with sublaminar wires.
On examination: He is in wheelchair with Foley's catheter. The
deltoids are 5/6 below the the upper limbs muscle power if 4/5 right
side and 3/5 left side. The left quadriceps muscle 1/5 , but all
muscles lower limbs are 0/5. There is analgesia from the hands ,
ulnar side of upper limbs and below. The patient is urging to remove
the construct to perform new MRI to see the condition of the spinal
cord.
The old wound refreshed and the Hartshill
rectangle with sublaminar wires were removed, Routine closure of the wound.
Smooth postoperative recovery,, but
neurologically the same. He was sent to the
ward.
FOLLOW UP
The patient came to the clinic 13-May-2004 with
new MRI done 03-March-2004 showing severe damage
of the spinal cord at C6 and 7 levels. The
patient was sent for ECS and EMG of the brachial
plexus, but he disappeared.
Comments
The patient escaped follow up and and it was
hard to plan for him alternatives to improve his
quality of life.
