43 YEARS EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 02-June-2002
complaining of LBP for 3 months and right sciatica
for 1 month with
numbness of the right L5 territory. MRI lumbar
spine performed 31-May-2002 showing huge
extruded disc L4-5 with right foraminal
occlusion.
On examination: The patient is limping with exaggerated scoliotic stance. SLRS was
35 degrees with
pain in right side. There is weak dorsi and
planterflexion right foot
3/5 with hypalgesia right S1 territory.
Right L5 foraminotomy with removal of the
extrusion lateral to the axilla and right sided
intradiscal cleaning L4-5. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot normalized.
He was sent to the ward.
Follow Up
The patient came to the clinic 22-June-2002
with clean wound and sciatica free and normal
power right foot with occasional numbness right
L5 territory.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
