The patient came 30-August-2006 complaining of LBP for 5 years. The
last 25 days, he got exacerbation of his LBP with left sciatica with
numbness of the anterior thigh. MRI lumbar spine performed
21-August-2006 showed extrusion of L2-3 disc with left upward
migration. On examination: there is weak dorsiflexion left foot
with hypalgesia of the anterior aspect of the thigh. He had OA both
knees.
Using imageintensifier the L2-3 level was identified and left
hemilaminectomy was performed and the extruded disc was removed in
several pieces. It was sequestrated and projecting lateraly to the
foramen. Inspection of the disc L2-3 revealed no local problems
mandating its removal. It was left intact.
Routine closure of the wound. Prompt postoperative recovery.
Comments:
1. It is mandatory to use imageintensifier, when working in high
level of lumbar disc extrusions to be sure, not violating the wrong
levels. |
