HUGE EXTRUDED DISC L3-4 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the Emergency of Shmaisani
hospital 18-August-2014
complaining of LBP for 10 days with left
sciatica.
MRI lumbar spine done 10-August-2014 showing
huge extruded disc L4-5 with left downward
migration and bulge L2-3.
On examination; the patient is in agonizing
pain, limping with exaggerated scoliotic stance,
using crutches to walk.
SLRS was 30 degrees with pain in the
left and 80 degrees in the right. There is weak
dorsiflexion left foot +3/5 and planterflexion
same foot 4/5.
The L3-4 level was identified by the C-arm.
Using MICRODISCXS system, left partial flavotomy
L3-4. The extruded disc was removed in several
pieces from the left lateral to the axilla. Left
sided intradiscal cleaning L3-4 disc space.
Routine closure of the wound.
Smooth postoperative recovery with normalization
of the lower of the left foot.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
is still not completely shallow.
