The patient came to the clinic 27-July-2002 complaining of LBP for 3 months with left sciatica after slipping from the table. MRI lumbar spine performed 02-June-2002 showing extruded disc L5-S1 with left foraminal occlusion.

On examination: The patient in agonizing pain, limping with exaggerated scoliotic stance. Positive cough sign. SLRS 80 degrees left side with pain. There is weak dorsi and planterflexion left foot-4/5.

Left S1 foraminotomy with removal of the extrusion lateral to the axilla. Left sided intradiscal cleaning L5-S1. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.

Smooth postoperative recovery. The power of left foot improved. She was sent to the ward.

The patient came to the clinic 11-August-2002 with clean wound and full power of the left foot.

The patient came to the clinic 07-February-2013 complaining of neck and four limb pain with morning stiffness. There was weak dorsi and planterflexion left foot 4/5. She was sent for investigations.

The patient then came 21-March-2013 with MRI cervical done 07-March-2013 showing bulge C5-6. Lumbar showing bulge L4-5 and L5-S1 with ESR 45 mm/h. The patient was treated conservatively.

The last time she came 18-September-2016 with MRI showing I degree Arnold-Chiari with bulge C5-6, 6-7. ESR still 45 mm/h. Conservative treatment was advised.

There is still an estimated postoperative recurrence around 7%.

The patient over the years will suffer from other condition not related for his surgery.
