21-DECEMBER-2015 AHLAM MUHAMED AL-QAYSI 33 YEARS
EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient was operated elsewhere 4 years ago
for PLD L4-5. The patient then came to the
Emergency Shmaisani hospital 20-December-2015
complaining of LBP with agonizing left sciatica
the last 10 days.
On examination: the patient is in agonizing pain
in bed. There is weak dorsi and planterflexion
left
foot 3/5. Positive cough sign and absent left
AJ. with hypalgesia left L5 and S1 roots
territories.
The patient was sent for investigations and MRI
done 20-December-2015 showing extruded disc
L5-S1 with left downward migration. Dynamic studies were normal.
Left S1 foraminotomy was
performed and the extruded downward migrating
extrusion was removed and left sided
intradiscal cleaning was achieved. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 msec
duration the left S1 root was achieved. Routine closure of the
wound.
Smooth postoperative recovery.
The power of left foot became normal. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 21st case using the PRF mode
with MultiGen. This procedure regained routine acceptance it will be
a usual part of the disc surgery.
