HUGE EXTRUDED DISC L4-5 RIGHT SIDE WITH DOWNWARD MIGRATION AND DROP
RIGHT FOOT.
Anamnesis
The patient came to the emergency of
Al-Shmaisani hospital with drop right foot and
agonizing sciatica right side for 10 days.
MRI of the lumbar spine performed 2 days ago
showed a huge extrusion of L4-5 with right
downward migration. The patient is a known
diabetic and he was put under observation for
his diabetic scale.
On examination: the patient cannot walk with
SLRS 20 degrees and drop right foot with
analgesia of the right L5 root territory.
Right L4-5 hemiflavotomy was done and the
extrusion was removed and the disc space of L4-5
was cleaned from the right side.
Routine closure of the wound and smooth
postoperative recovery with improvement of the
motor power right foot.
Comments
The extrusion was reaching the posterior
aspect of the root. It was removed and to keep the root
under visual control, the piece was removed in several
fragments.
Considering that the intradiscal material was
friable, meticulous cleaning of the disc space was performed to
decrease the rate of recurrence.
