POSTTRAUMATIC FRACTURE-DISLOCATION C3-4 WITH INTRADISCAL INJURY TO C4-5.
Anamnesis
The patient suffered RTA this morning without
loss of consciousness with severe neck pain. He
was admitted initially to Al-Bashir hospital and
CT-scan of the brain was normal and CT-scan of
the neck showed fracture dislocation C3-4.
Cervical X-rays also confirmed this. The patient
then was brought to Shmaisani emergency and MRI
of the cervical spine showed the fracture
dislocation with intradiscal injury of C4-5.
On examination, the patient in bed with soft
collar fully oriented with severe neck pain.
There weak left upper and lower limbs.
Discectomy of C3-4 was performed with insertion
of Medtronic compact PEEK anatomical cage
14x11x4 mm. Intradiscal decompression of C4-5.
Using Medtronic Atlantis plate 2 level 42.5 mm
with 5 polyaxial screws 4x13 mm to C3 C5 and
right side of C4 and one screw 4.5x13 mm to the
left side of C5, fixation and reduction of
C3-4-5 was achieved. All stages of surgery were
guided with the C-arm Routine closure of the
wound.
Smooth postoperative recovery. The power of the
left side became better.
Comments
The patient must be fused at these level
to prevent future escalation of C4-5 intradiscal torn
material.
