EXTRUDED DISC L4-5 WITH LEFT DOWNWARD MIGRATION.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is not
completely collapsed.
Anamnesis
The patient came to the clinic
02-June-2003 complaining of LBP with left
sciatica for 3 weeks with positive cough sign.
MRI lumbar spine performed 27-May-2003 showing
extruded disc L4-5 with left downward migration.
On examination: the patient in agonizing pain,
limping with exaggerated scoliotic stance. SLRS
was 85 degrees both sides without pain. There is
hypalgesia left S1 root with almost drop left
foot with weak planterflexion same foot 3/5.
Left L4-5 hemiflavotomy with
extended left L5 foraminotomy. The extruded disc of
L4-5 was removed from the left and left sided
intradiscal cleaning of L4-5 disc space was achieved
until the root was hanging free. The patient was put
in Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic 18-June-2003 with
clean wound and still having weak dorsiflexion
left foot 3/5. Sensation improved.
