15-DECEMBER-2007 AHMAD REZIQ MAHMOUD 38 YEARS LEFT UPWARD MIGRATING DISC
L4-5 WITH SECONDARY SEVERE LCS L4-5.
Anamnesis
The patient came to the clinic 13-December-2007
complaining of left sciatica and LBP for 7
years. Exacerbation of the sciatica the last 20
days with numbness left foot. He has dripping of
urine after voiding the last 6 months. MRI done
24-November-2007 showed an extruded disc L4-5
with left upward migration resulting in severe
lumbar canal stenosis at L4-5.
The patient had weak dorsiflexion left foot 4/5
with hypalgesia left L5 territory.
2/3 left L4-5 flavotomy was performed and
foraminotomy of left L5 root was achieved. The
upward migrating extrusion was removed, after
then the disc space was cleaned meticulously
from the left. Epidural fat transfer was
performed to cover the root.
Routine closure of the wound and smooth
postoperative recovery.
Comments
The patient has extruded disc with severe
segmental stenosis at L4-5, for what wide exposure and
decompression was performed to eliminate all the compressing
elements.
The recurrence rate in this case is around
the average - 7% because, the disc space is still high and the
defect in the annulus fibrosis is wide.
