Comments
The recurrence rate in cervical disc is
zero.
Anamnesis
The patient came to the clinic 12-March-2003
complaining of numbness both hands and both legs
below the umbilicus with weak both upper limbs
with pain neck when rotating the head to all
direction. Dripping urine for 2 weeks. The
patient is hypertensive. MRI cervical spine
performed 08-March-2003 showing severe canal
stenosis C3-4, more from old extrusion C3-4 with
gliosis of the spinal cord.
On examination: the patient has neck pain
exacerbated with turning the head to all
directions. There is weak all muscles upper
limbs, more the right. All reflexes are
exaggerated with Hoffmann sign positive both
sides. Both lower limbs weak more the left with
spastic pattern and exaggerated deep reflexes
and Babinski positive both sides with clonus
both sides.
The patient was sent for investigations. There
is small osteophyte at the left corner of C5-6.
Dynamic studies showing overmobility of C4-5.
Transnasal intubation. Through
anterior approach,
discectomy C3-4 was achieved under the control of
C-arm. Using drilling the C3-4 calcified posterior
longitudinal ligament until the dura was seen under
the field. Routine closure of
the wound
Smooth postoperative recovery.
The radicular pain decreased and the power of four
limbs improved. He was sent to the ward.
Follow Up
The patient came to the clinic 01-April-2003
with clean wound with improvement of the power
of the four limbs with normalization of
sensation all over. The pathologic reflexes
still present.
