RECURRENCE OF PLASMOCYTOMA WITH KYPHOTIC DEFORMITY AT D6-7.
Comments
The recurrence of plasmocytoma mostly due
to incomplete resection in the first surgery. In this second
surgery resection even with healthy tissues was achieved.
Anamnesis
The patient was operated by me
10-March-2002 for plasmocytoma. The patient
put in observation and the last MRI showing
recurrence of the plasmocytoma with kyphotic
destruction of D6 and D7.
On examination: the patient has dorsal pain with
kyphotic spine. But neurologically he was free.
The old wound was refreshed and
the Luque with sublaminar wires were removed. The
tumor was attacked from both sides and drilling of
the bony part with removal of the tumor masses was
achieved until radical resection of the mass and the
bony compression were eliminated. The Luque
rectangle was bended more straight and sublaminar
wires were applied for more vertebrae above and
below. The roots were preserved and attempt to avoid
condition to be compressed was undertaken. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient was advised to receive radiotherapy
to prevent recurrence.
