20-JULY-2014 AKRAM ISSA AL-SAADY 39 YEARS LEFT
EXTRAFORAMINAL EXTRUSION L4-5 DISC.
Anamnesis
The patient came to the clinic 20-July-2014
complaining of left sciatica for 10 days down to
the left knee and medial aspect of the left
foot. MRI lumbar spine done 17-July-2014 showing
left extraforaminal extrusion L4-5.
On examination; the patient using 2 crutches to
walk, limping with exaggerated scoliotic stance.
SLRS was 80 degrees with pain in the left. The
left KJ is absent. There is weak dorsiflexion
left foot 3/5.
Using C-arm, the L4-5 level was identified.
Midline incision. Foraminotomy left L5 root. The
extraforaminal extrusion was removed in one big
piece. Left sided intradiscal cleaning of L4-5
disc space. Routine closure of the wound.
Smooth postoperative recovery. The power of left
foot became normal and the left sciatica
disappeared.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space height is still
not completely shallow.
