06-AUGUST-2015 ALAWI HIZAM ABDALLA 55 YEARS
CHRONIC BACK PAIN WITH RIGHT SCIATICA DUE TO BULGE L4-5 AND L5-S1.
Anamnesis
The patient came to the clinic 19-June-2014
complaining of LBP with right sciatica for 2
months. He was limping with no scoliotic stance.
SLRS at that time was 40 degrees with pain right
side. There was weak dorsiflexion right foot
-4/5. MRI lumbar spine performed 19-June-2014
showed bulge L4-5 and L5-S1. Dynamic studies
were negative for over mobility. He had gout for
what pain killers and Zyloric were prescribed.
The patient then came several times with neck
and LBP and pelvis MRI was done 26-March-2015
which was normal. The patient then came
04-August-2015 telling that he still has LBP
with right sciatica, but he was not limping with
full power of both feet. SLRS was 50 degrees
both sides with pain right side. It was
suggested to the patient to undergo
radiofrequency ablation using MultiGen and it
was explained to him that the LBP can be treated
but concerning the sciatica remain under
question. He agreed.
Using the C-arm the L5 and S1 points were
localized both sides and sensory test and motor
test was applied for all points. The patient
during sensory test claimed some desirable
feeling. During motor test the canula was
vibrating at 1.5 V. All 4 sites were ablated
thermally using 80c and 90 sec. The patient was
all the time interrogated and he is free of pain
at the end of the procedure.
The patient was discharged after 1 hour.

Comments
The patient has chronic LBP which is not
responding to medication and surgical intervention is not
needed. RF ablation was the best choice in this case.
