15 YEARS PROGRESSING RIGHT EXTRADURAL HEMATOMA RIGHT TEMPORO-PARIETAL
REGION.
Anamnesis
The patient admitted to Shmaisani hospital after
suffering bicycle accident 30-October-2002 to the ICU
conscious, moving four limbs with GCS 15/15.
Urgent CT-scan showed small hematoma right
temporo-parietal region with fracture of the
right temporo-parietal bones.
The next day the patient started to show
deterioration of his condition and repeat
CT-scan showed enlargement of the extradural
hematoma with mild shifting of midline
structures, but still not in conning.
Osteoplastic craniotomy over the
right temporal region and the hematoma was evacuated
and strict hemostasis of the source of the bleeding.
The bone flap returned to place. Routine closure of
the wound
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic 29-March-2003
complaining of intermittent headache left side
with episode of epi attacks, which took place 1
month after accident. Epanutin started and sent
for investigations.
The patient came 05-April-2003 with MRI of the
brain done 01-April-2003, which was normal with
EEG showing abnormal epi-activity, more to the
right. The patient was asked to be followed for
that, but he disappeared.
Comments
It is better to avoid seeing the conning,
but anticipate it and evacuate the hematoma to prevent
further damage to the brain tissues.
