Comments
The direct anastamoses is the best option
for recovery instead of putting a sural graft.
It will be beneficial to know what
happened to the patient and follow his recovery. He was from
Palestinian Intifada and for political reasons disappeared.
Anamnesis
The patient came to the clinic 03-March-2003
complaining of non-functioning right median and
ulnar nerves after gun shot. The patient was
operated by Dr. Kamel Afifi in December-2002.
On examination: the patient has complete
function of the MCN and radial nerve, but the
right median and ulnar nerves are not
functioning.
Projection incision above the
lesion and both median and ulnar nerves exposed and
by stimulation did not respond. Exposure of the
distal continuity was performed and both nerves were
dissected toward the maximum scar. Both nerves had
neuroma, which was bisected until good fibrils were
seen. Directed anastamoses was performed using 4
zero nylon and have maximum proximity, the
ulnar nerve was transposed anterior. Routine closure
of the wound and a cast was applied to avoid
unintentional stretching of the elbow .He was sent to the ward.
