LUMBAR HUGE MENINGO-RADICULOCELE.
Anamnesis
The patient was brought to me with huge upper
lumbar meningo-radiculocele 8 days age with
normal neurologic status.
On examination: The patient is neurologically
free.
Circumferential incision around
the neck of the meningocele with inspection of the
most near part of the neck near the dura. Some of
the running nerves were bisected and preserved. The
neck was bisected and the mass removed with closure
of the dura. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic twice:
17-March-2002 and 25-March2002 with H/C 35 cm
and the parents were told to perform brain
CT-scan after 1 month, but they disappeared.
Comments
The meningo-radiculocele in this case was
not challenging, since the neural structures were not
involved.
Follow up is mandatory, to rule out
progression of hydrocephalus and rule out tonsilar herniation,
but the parents escaped.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
