LUMBAR CANAL STENOSIS L4-5, L5-S1.
Anamnesis
The patient a gynecologist, came to the clinic
15-April-2002
complaining of LBP with left sciatica
for 4 years with exacerbation when walking. He
cannot walk more than 10 meters. MRI performed 10-April-2002 showing
severe LCS L4-5 and L5-S1.
On examination: The patient is limping with exaggerated scoliotic stance
with left sciatica. SLRS was 90 degrees without
pain both sides. There is weak dorsi and
planterflexion both feet
-4/5 with hypalgesia both L5 and S1 territories
both feet.
Decompressive laminectomy L4-5, upper edge of
the sacrum and lower half of L3 with bilateral
extended L5, S1 foraminotomy with removal of all
compressive elements . The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the feet improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 03-June-2002
with clean wound and sciatica free and normal
power both feet.
The patient then came 23-April-2003 telling that
he suffered RTA with performed CT-scan of
the spine and MRI lumbar area showing wedge
fracture L3. He was advised to be treated
conservatively.
Comments
The sooner the surgical decompression in
progressive lumbar canal stenosis, the better the
postoperative outcome.
