YEARS EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 16-June-2002
complaining of LBP with left sciatica for
2 years. She cannot walk more than 50 meters.
She is a known diabetic for 2 years. She has
cataract right eye since childhood.
On examination: The patient is limping with exaggerated scoliotic stance.
SLRS was
20 degrees with
pain in left side and 80 degrees without pain in
the left. There is weak dorsi and
planterflexion left foot
3/5.
The patient was sent for investigation: MRI
lumbar spine performed 19-June-2002 showing very
large extrusion L5-S1 with left foraminal
occlusion and migrating upward.
Left S1 foraminotomy with removal of the
extrusion lateral to the left axilla and left sided
intradiscal cleaning L4-5. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot normalized.
She was sent to the ward.
Follow Up
The patient came to the clinic 06-July-2002
with clean wound and sciatica free and normal
power left foot. SLRS was 60 degrees with mild pain
in the left.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
