18-SEPTEMBER-2003 AMAL SULAYMAN AHMAD 37 YEARS HUGE LEFT
EXTRUDED DISC L5-S1.
Anamnesis
The patient came to the clinic
16-September-2003 complaining of LBP with left
sciatica for 1 month with radiation down to the
left S1 root territory. MRI lumbar spine done
07-September-2003 showing extruded disc L5-S1
with left downward migration.
On examination: the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 45 degrees left side with pain. There
is weak dorsiflexion left foot with hypalgesia
left S1 root territory.
Left sided L5-S1
hemiflavotomy with foraminotomy left S1 root.
The extruded disc was removed lateral to the
axilla and left sided intradiscal cleaning of
L5-11 space was achieved. The epidural fat was
transferred around the left S1 root to minimize
postoperative adhesions. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
left foot normalized.
FOLLOW UP
The patient
came to the clinic 06-October-2003 with clean wound
and the power and sensation of the left foot
became normal. SLRS was 80 degrees with tightness.
Comments
There is still an
estimated postoperative recurrence around 7%
because the disc space is not completely shallow.
