WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated by me
28-January-2012 for extruded disci C3-4, 4-5
and 5-6 and she improved for the upper limbs
with persisted hypalgesia below C8 left side
with weak dorsiflexion both feet 3/5 and
planterflexion left foot -4/5. The patient then
came 19-October-2014
complaining of LBP with right sciatica for
4 months with further deterioration last 4
weeks. The patient is a known hypertensive for
15 years.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS 80 degrees
with pain both sides. There is weak dorsiflexion
right foot 3/5. Dorsiflexion left 4/5 with full
power of planterflexion both feet. There is
hypalgesia below the knees both sides.
MRI of the lumbar spine done the same day,
showing extruded disc L4-5 with right foraminal
occlusion. Dynamic studies were normal and the
construct in the cervical spine implanted
earlier was acceptable.
Using C-arm. the L4-5 level was identified.
Bilateral foraminotomy of L5 roots was achieved.
The extruded disc was removed lateral to the
axilla. Inspection of the left side was
uneventful. The disc space was very shallow.
During the procedure ILLICO retractor with
illumination was used. Routine closure of the
wound.
Smooth postoperative recovery. The power of the
right foot became normal.

Comments
The patient still has an estimated
postoperative recurrence below 7%, because the disc space
is completely shallow.
