YEARS HUGE EXTRUDED DISC C6-7 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 24-February-2014
complaining of neck and right shoulder pain for
2 weeks of sudden onset with agonizing character
with numbness of the index and thumb right hand.
She cannot sleep due to pain.
On examination; the patient has pain with
limitation of neck movement to all directions.
The right biceps, hand extension and grip weak
4/5. The right triceps 3/5. There is hypalgesia
of right C6, roots territories.
MRI of the cervical spine done the same day
showing huge extruded disc C6-7 with right
foraminal occlusion with bulge C5-6.
Using C-arm, the C6-7 level was identified.
Discectomy C6-7 done with removal of the
extrusion until the dura was seen at the most
posterior aspect of the disc space. Vitatech
cage 6.5x16 mm inserted to the disc space. One
level plate 26 mm length was used to fuse C6-7
with 4 screws 16 mm length.
Smooth postoperative
recovery. The power of the right upper limb
became normal. The agonizing radicular pain
disappeared.
Comments
The patient has bulge C4-5 and C5-6. If to
include C5-6 with fusion, more stress will
influence the C4-5, for what only one level
fusion was achieved.
