The patient started to complain of severe LBP with bilateral sciatica for 3 months. The patient was seen immediately in the operating table with agonizing pain and inability to set down. Weak dorsiflexion both feet more the right was noticed. Bilateral L4-5 flavotomy was performed and the huge extrusion was attacked below the right L5 axilla and it required patience to remove it due to huge dimensions and to avoid traumatic traction during its removal. Power both feet recovered promptly 15 min. after surgery.
