The patient came to the clinic 05-October-2002 complaining of LBP with right sciatica for 5 years. She was seen by me at that time and advised for surgery, but she refused. The the last 6 months exacerbation of LBP. MRI lumbar spine done 28-July-1997 showing extruded disc L5-S1 right and bulge L4-5. MRI repeated 12-June-2002 showing the same data.

On examination: The patient is limping, has LBP with exaggerated scoliotic stance. There is weak dorsiflexion right foot 3/5 and planterflexion right foot 4/5, hypalgesia right S1 territory and SLRS was 45 degrees with pain right side. The right AJ is absent.

Right S1 foraminotomy with removal of the extrusion lateral to the axilla and drilling of the osteophytes with right sided intradiscal cleaning of L5-S1 disc space. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.  Routine closure of the wound.

Smooth postoperative recovery. The power of the right foot normalized. She was sent to the ward.

The patient came to the clinic 17-October-2002 with clean wound and normal power and sensation right foot. SLRS became 80 degrees without pain in the right.

The patient then came 01-May-2006 telling that after delivery, got left sciatica for 6 months . MRI lumbar spine performed March-2003 ruled out recurrence. She was limping in agonizing pain with almost drop left foot and hypalgesia left L5, S1 territories. She was given medications and sent for investigations, but disappeared.

The patient then came 05-May-2020 telling that the last 15 days got right sciatica. She was limping. Motor power left foot 5/6, weak right dorsiflexion 4/5. She was sent for investigations and another time disappeared.

The estimated postoperative recurrence is around 7% because the disc space is still not shallow.
