Comments
The cervical disc not recurring.
The various neural elements of the spinal
cord recover at different speed. Micturition needs time to
recover.
Anamnesis
The patient came to the clinic 14-April-2002
complaining of numbness left upper limb for
three years and numb both legs with weak of both
hands for 2 weeks with incontinence. MRI
cervical spine performed 06-April-2002 showing
very huge extruded disc C3-4, more to the left,
with severe spinal cord compression and gliosis
of the spinal cord. The patient is hypertensive
for one year, in medication.
On examination: The patient walking with help.
There weak both upper limbs more profound in the
left with Rossolimo's sign positive in the left.
Transnasal intubation with
anterior approach to the C3-4 level with check
with C-arm. Discectomy of C3-4 with removal of
the huge extrusion and removal of the posterior
longitudinal ligament until the dura was seen at
the posterior border of the disc space cavity. No CSF leak. Routine closure of the
wound.
Smooth postoperative recovery.
The power of the upper limbs improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 27-April-2002
with clean wound with no sensory or motor
deficits, but still having problems with
micturition. The patient was given medications
to promote recovery of the spinal cord and she
disappeared.
