28-MARCH-2015 ANAN ABDEL-RAHEEM AL-QADI 45 YEARS
EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 23-March-2015
complaining of LBP with left sciatica for 2
years intermittent course with exacerbation last
month.
On examination, the patient is limping in
agonizing pain with exaggerated scoliotic stance. SLRS
50 degrees in the right and 25 degrees
with pain in the left
side. There is weak dorsiflexion left foot 3/5
and hypalgesia left S1 territory.
The patient was sent for investigations and MRI
done 24-March-2015 at Tachasusi hospital showing huge extruded disc L5-S1 with
left downward migration. Dynamic studies were
incomplete.
Foraminotomy left S1 root. The extruded
disc L5-S1 was severely compressing the root
that a tear got place during dissection. It was removed lateral to the axilla
after partial decompression of the disc space.
Right sided intradiscal cleaning L5-S1.
The dural tear of the S1 root was repaired using
6 zero nylon. Check for CSF leak using Valsalva
maneuver and positioning the head up was
negative. There was no CSF leak even before
repair. The subaxillary region was inspected. Routine closure of
the wound.
Smooth postoperative recovery. The power of left
foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not shallow.
