30-JUNE-2004 ARABIEH YOUSEF AL-TURK 37 YEARS
RECURRENT DISC L4-5 RIGHT SIDE.
Anamnesis
The patient came to the clinic 28-June-2004
complaining of LBP with right sciatica, for 1 month. MRI lumbar spine
performed 30-May-2004 showing huge recurrence
L4-5 more to the right. She was operated
elsewhere for PLD L4-5 with left sciatica.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsi and planterflexion right foot 3/5. SLRS was
40
degrees with pain right side.
Foraminotomy right L5 root. Right sided removal of the extrusion and
right sided
intradiscal cleaning L4-5 disc space.
The huge piece of extrusion was removed
subaxillary in one very big piece. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A fat graft with
pedicle was
achieved from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the right foot normalized.
FOLLOW UP
The patient
came to the clinic 10-July-2004 with clean wound and no motor,
nor sensory deficit. SLRS was 70 degrees both sides
with pain.
Comments
There is still an
estimated postoperative re-recurrence around 7%,
because the disc space is still not completely
collapsed.
