HUGE EXTRUDED DISC L4-5 MORE TO THE RIGHT.
Anamnesis
The patient came to the clinic 03-October-2004
with LBP for 6 months and right sciatica for 1
month. MRI lumbar spine performed
21-September-2004 showing huge extruded disc
L4-5 more to the right.
On examination: the patient is limping with
exaggerated scoliotic stance with agonizing
pain. SLRS was 70 degrees with pain in the
right. There is weak dorsiflexion right foot
4/5.
Right L4-5 hemiflavotomy with foraminotomy right
L5 root was achieved. The extruded disc was
removed lateral to the axilla. The patient was
put in Reverse Trendelenburg position with
Valsalva maneuver and hyperventilation. No CSF
leak. Routine closure of the wound.
Smooth postoperative recovery and the power of
the right foot normalized and the patient
was sent to the ward.
Follow Up
The patient came to the clinic 04-December-2004
with clean wound and normal power of the right
foot.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
