19-FEBRUARY-2015 ASEM ABDALLA UMAR 63 YEARS HUGE
EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 09-February-2015
complaining of left sciatica down to all toes
left foot with numbness of the II-V toes left
foot. The patient had similar episode 30 years
ago and resolved. MRI lumbar spine performed
17-January-2015 showing extruded disc L4-5 with
left foraminal occlusion with bulge L5-S1. The
patient is a known diabetic for 5 years and
hypertensive for 10 years, underwent Labcholi 8
months ago. The patient receiving Baby aspirin.
On examination, the patient is in agonizing pain
limping with exaggerated scoliotic stance with
SLRS 15 degrees with pain left side with weak
dorsiflexion left foot 0/5 and right foot 3/5.
There is hypalgesia left L5 and S1 roots.
The patient sent for investigations: MRI lumbar
spine done 18-February-2015 showing huge
extruded disc L4-5 with left foraminal occlusion
and bulge L3-S1. Dynamic studies performed
before surgery ruling out instability.
Using the C-arm, the level of L4-5 was
identified. Foraminotomy left L5 root with
subaxillary removal of the huge extrusion. Left
sided cleaning L4-5 disc space. Routine closure of the wound.
Smooth postoperative recovery.
The power of the both right became normal
immediately after surgery, but the left foot
dorsiflexion slightly improved..
Comments
The patient still has an estimated
postoperative recurrence around 7%.
