EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient
came to the clinic 03-November-2014 complaining of LBP
for 16 years with
exacerbation of the pain and right sciatica for 2
weeks with numbness of the III-V toes right
foot. MRI lumbar spine done 29-October-2014
showing huge extruded disc L4-5 with right
downward migration.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 75
degrees with pain in the right. The AJ is absent
in the left? Weak dorsiflexion right foot 4/5.
Using C-arm and MICRODISCXS system, the L4-5
level was identified and the tubular system
introduced from the right. Foraminotomy of the
right L5 root. The extruded huge disc with
downward migration was removed in one piece
lateral to the axilla. Right sided cleaning L4-5
disc space.
Routine closure of the
wound.
Smooth postoperative recovery.
The power of the right foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not completely shallow.
The MICRODISCXS system is defective and
has many disadvantages. This conclusion came out after more
than 20 surgeries were performed using such system. The only
advantage that it is cheap.
