Anamnesis
The patient a Bahraini citizen, came to the clinic

complaining of LBP with left sciatica down to
the L5 root without improvement for 3 years
after falling down Right sciatica for 2 months
and inability to walk more than 10 meters.
Diabetic for 3 years, hypertensive for 36 years.
CABAG done 17 months ago.
On examination: The patient has LBP with
exaggerated scoliotic stance. There is weak dorsiflexion both feet
3/5 and planterflexion right foot 4/5, both
quadriceps femoris 4/5.
She was sent for investigations:
MRI lumbar spine, which was done
26-September-2002 showing severe lumbar canal
stenosis L2-3, 3-4 4-5 and L5-S1. Cardiac
consultation was uneventful.
Decompressive laminectomy L3,4,5, upper edge of the
sacrum and lower third of L2 with foraminotomy both
L3,4,5 and S1 both sides was achieved. Inspection of
the disci was uneventful. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.

Smooth postoperative recovery.
She was sent to the ward.
Follow Up
The patient came to the clinic 09-October-2002
with clean wound and still full power both feet. Walking improved.
Comments
The sooner the surgical decompression in
progressing lumbar canal stenosis the better the
post-surgical outcome.
