55 YEARS RIGHT FRESH UPON OLD LEFT EXTRUSION L5-S1 DISC.
The patient came
to the clinic 13-September-2006 complaining of LBP for 15 years with
intermittent left sciatica. The last 2 months progressed right
sciatica. The patient claim, that he is improving, but in the
clinical examination, he had weak dorsiflexion both feet and SLRS
was 70 degrees in the left with exaggerated scoliotic stance. The AJ
was absent in the left side.
MRI lumbar spine was performed
13-September-2006 which showed the old left sided extrusion L5-S1
with fresh extrusion in the right side.
Bilateral L5-S1 flavotomy with foraminotomy both S1 roots was
done. The fresh extrusion was removed lateral to the right
axilla . The left extrusion was bony hard with the disc space very
narrow. It was decided to drill out some of the osteophytes, which
could irritate the running above both roots. Routine closure with
prompt postoperative recovery.
Comments:
1. The recurrence in this case must be minimal, if not zero,
because the disc space is completely absent and there is no
intradiscal material to cause recurrence.
