EXTRUDED DISC L4-5 MORE TO THE LEFT.
Anamnesis
The patient came to the clinic
07-November-2003 complaining of LBP with left
sciatica with numbness left foot for 2 weeks. MRI
lumbar spine done 28-October-2003 showing huge
extruded disc L4-5 left foraminal occlusion.
On examination: the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 30 degrees left side with pain. There
is weak dorsi and planterflexion left foot with
hypalgesia left L5 and S1 root territories.
Decompressive laminectomy
L4-5. Foraminotomy left L5
root was achieved. The
extruded disc material of L4-5 was removed from the
left
side with left sided intradiscal cleaning L4-5 was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the subcutaneous layer and
covered the exposed parts of the root. Routine
closure of the wound. Smooth postoperative
recovery. The power of the left foot became
normal.
FOLLOW UP
The patient came to the clinic
22-November-2003 with clean wound
and no sensory, nor motor deficit.
Comments
There is still an
estimated postoperative recurrence around 7%
because the disc space is still not completely
collapsed.
