EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The
patient , a Libyan citizen through the Majlis
Al-Intikaly, came to the clinic 07-January-2012
complaining of LBP with agonizing left sciatica
after falling down from a moving car
20-March-2011.
On
examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 70
degrees with pain in the left. There is weak
dorsi and planterflexion left foot 4/5. There is
hypalgesia left L5 and S1 roots.
MRI
lumbar spine done 31-December-2011
showing extruded disc L4-5 with left foraminal
occlusion and downward migration.
Left L5 foraminotomy
with partial flavotomy L4-5. The compressed root
and surrounding dura are lacking the epidural
fat. The extruded disc material was removed
lateral to the axilla and meticulous cleaning of
L4-5 was performed from the left.
Routine
closure of the wounds. Smooth postoperative
recovery with improvement of the power of the
left foot.

Comments
The patient still having estimated postoperative
recurrence rate more than 7%, because the disc
space height still high.
