AZIZA SULAYMAN SALEM ABDEL-RAHMAN 46 YEARS EXTRUDED DISC L4-5 WITH
LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 12-March-2004
complaining of LBP with left sciatica for 11
month with numbness of the left L5 and S1
territory.
On examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 40
degrees left side with pain. There is weak dorsi and planterflexion
left foot 4/5 with hypalgesia
left L5 and S1 territory.
The patient was sent for investigations and MRI
lumbar spine performed 14-March-2004 showed
huge extruded disc L4-5 with left
downward migration.
Foraminotomy left L5 root with
removal of the extrusion lateral to the axilla. After
that, the root became lax and left sided
intradiscal cleaning L4-5 disc space was done.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the subcutaneous layer and
covered the exposed parts of the root, to
minimize postoperative scarring. Routine closure
of the wound. Smooth postoperative recovery. The
power of the left foot normalized.
FOLLOW UP
The patient came to the clinic 29-March-2004
with clean wound and SLRS 70 degrees left side
without pain and normal power left foot and
sensation.
The patient then came 05-July-2008 telling after
bending 4 days ago got LBP with right sciatica.
MRI done yesterday showing no recurrence at
L4-5, but there is right extruded disc L3-4
far-lateral soft in consistency. On examination
the right foot was almost drop with SLRS 5
degrees right side. The patient was advised to
keep in conservative treatment.
The patient then came 04-August-2008 with the
same condition and she was advised to undergo
surgery and given admission, but she
disappeared.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
